NYCkayaker Death, Dry Suits, Liability and Barriers

 Lets say a 23 year old athletic, strong, healthy guy/gal who was  too old
for their parents health care. They were working at part time or starting
out full time at one of these very common mega corporations that classify
everyone as freelancers to avoid benefits.  Lets say they tore their rotator
cuff while kayaking, rescuing a little girl who fell off a pier or cliff
while trying to catch her teddy bear. 
He/she kayaker has no health insurance to pay for a good surgeon and
hospitalization, PT .
Should they suffer the rest of their life and be forever crippled by making
a snap decision to rescue, without thinking of the consequences?
Some injuries if not corrected quickly can cause permanent lifetime damage.
